How your consent form is handled
This page covers the consent, release and aftercare form you fill in at your appointment with Tevin Greene Tattoos.
What the form collects
- Your name, date of birth, phone number, email address and home address.
- The name and phone number of an emergency contact.
- Your answers to the health questions.
- The tattoo you are getting and where it goes.
- A photo of the front and back of your photo ID, and the type of ID.
- Your agreement to each section, your typed signature, and the date and time you signed.
- Your choices about photos of your tattoo and about being contacted afterwards.
Why it is collected
- To confirm you are 18 or older. North Carolina law does not allow tattooing anyone under 18.
- To record your consent before your tattoo begins.
- So your artist knows about anything that could affect your tattoo or how it heals.
- To reach you, or your emergency contact, about your appointment.
Where it goes
When your artist presses Send, your form and the two ID photos are emailed to the consent inbox of Greeneworks Tattoo, the studio where Tevin tattoos. The website does not keep a copy.
You get a receipt by email. The receipt has no ID photos and no health answers.
The ID photos are made smaller on the phone or tablet before they are sent. Two services carry the form on its way: the website’s host (Vercel) and an email delivery service (Resend).
The form page has no advertising or tracking tools on it.
Who sees it
The shop and the artist doing your tattoo. Your information is not sold, and it is not shared with anyone for advertising.
The shop keeps signed forms as part of its client records.
Contact after your visit
- Email: if you leave the email box ticked, the shop may email your aftercare steps and a request for a review. Untick it and it will not.
- Text messages: only if you tick the text message box. Reply “stop” to any text to end them.
- Photos of your tattoo are used only the way you chose on the form.
